Children and Adult Case Manager

Description: Provides services to participants living in the community with persistent mental health challenges, in addition to individuals with co-occurring conditions. After assessing needs, the case manager helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant’s goals for community living.

Hours: F/T, Monday-Friday, 40 hours

Facility Location: Philadelphia, PA 19131

Education Requirements:
• Bachelor’s Degree in related mental health field (Psychology, Sociology, Criminal Justice, etc.) + at least one year verified Case Management experience

Additional Requirements (for all BH positions):
• Updated resume
• 2 forms of identification (preferably driver’s license and SS card)
• 2 professional references that we may contact for previous employment feedback
• 2 professional references that we may contact for employment verifications (preferably HR)
• Degree(s)
• Transcripts
• First Aid card (must be current)
• CPR card (must be current)
• Physical (within 1 year)
• TB test results (TB within 1 year or chest xray within 2 years)
• Certificates/trainings relating to position
